Patient Safety Incident Response Policy and Plan
Optegra is pleased to present our Patient Safety Incident Response Policy and Plan (PSIRP), which is fully aligned with the NHS Patient Safety Incident Response Framework (PSIRF). This plan outlines our organizational approach to developing and maintaining robust systems and processes for effectively responding to patient safety events at both the local and national levels.
At Optegra, we are committed to learning from patient safety incidents and continuously improving the quality of care we provide. Our new approach emphasises the importance of refining systems, processes, and behaviours to achieve a sustained reduction in risk. Key elements include:
1. Enhancing Optegra’s response to incidents through the implementation of PSIRF.
2. Leadership and championing in patient safety improvement initiatives driven by the Quality Improvement Lead Team (QILT)
3. Fostering a positive safe culture.
This policy supports and promotes the four key aims of the PSIRF:
- Compassionate engagement and involvement of those affected by patient safety incidents.
- Application of a range of system-based approaches to learning from patient safety incidents.
- Considered and proportionate responses to patient safety incidents and safety issues.
- Supportive oversight focused on strengthening response system functioning and improvement.
